An Advanced Care Directive includes specific directions for medical treatment. It also includes specific directions for end of life decisions. It is an important way of directing others of your specific wishes about healthcare and treatment, should you find yourself seriously ill or injured and unable to make decisions for yourself.
An Advance Care Directive can only be made by an adult over 18 years with decision-making capacity, and if valid must be followed. Health Professionals can not override a valid Advance Care Directive. This document works in conjunction with your Enduring Guardianship Document.
